Social Determinants of Health Considerations

Patients need to be assessed for the following at their initial visit, annually, and at times of life transitions.

Assess financial burden of testing and treatment.
Clinician actions
  • Discuss costs of testing, treatment, and insurance status.
  • Engage social worker and financial assistance when appropriate.
  • Referral to nonprofit and epilepsy syndrome-specific advocacy organizations which may offer financial support programs.

Assess for transportation challenges.
Clinician actions
  • Inquire about transportation for appointments or medication refills.
  • Assess for mail-order medications if appropriate.

Re-evaluate medication when patient’s comorbidities or condition changes.

  • Evaluate for new health conditions or changes including pregnancy.

Assess for life transitions that may impact quality of care.
Clinician actions
  • Discuss important life transitions (independent living, pregnancy, puberty, divorce, etc.) and start planning early to identify necessary changes to the treatment plan, as well as finding a new caregiver and/or providers.

Assess for concerns regarding diet, weight, nutrition, and food and housing insecurity.
Clinician actions
  • Refer to community services and programs.
  • Refer to nutrition and diet counseling.

Assess for understanding of diagnosis and treatment instructions.
Clinician actions
  • Ensure culturally competent care and provide adequate interpreter services and educational materials in the patient’s primary language.
